Job Summary:
Responsible for the Life Enrichment (LE) support and well-being of Sage Living residents with an emphasis on residents living in the Memory Support neighborhood (Wildflower Way), through the development and implementation of life enriching activities, programs, and events that provide meaningful and enriching opportunities that create a sense of “success and value” for residents.
Essential Functions:
1.
Program Planning
Participates in planning LE activities for Wildflower Way and other Sage Living residents.
Develops a monthly program calendar in collaboration with the Life Enrichment Director with consideration of residents strengths.
Responsible for ensuring the calendar includes physical activities, cognitive stimulation, social activities, sensory stimulation, self-care (maintenance of independence), comfort activities, community activities, cultural events, and enter-generational activities.
Develops activities that meet the needs of residents that can be implemented by staff seven days a week.
2.
Program Coordination/Implementation
Coordinates and implements LE activities for all Sage Living neighborhoods.
Collaborates with neighborhood teams to optimize resources and support for the residents.
As needed, provides one-on-one activities and personalized activities geared toward residents' former lifestyles and interests in as stress-free an environment as possible.
Assists with LE documentation and communication needs such as: maintaining and distributing daily programming lists to neighborhoods and recording residents' activity participation; monitoring residents' involvement in and responses to LE activities and programs; assisting in creating and distributing the monthly newsletter; and assisting in creating fliers and/or signage for neighborhood programming and events.
Provides communication to the neighborhood nursing and rehab teams regarding daily events, as appropriate, to assure optimal participation by residents.
Includes the leadership team in communications for special LE events.
3.
Resident Strength-Based Care Planning
Collaborates with the Life Enrichment Director to determine optimal program and activity involvement for residents of all neighborhoods, based on their needs, strengths, and interests.
Provides electronic documentation of residents' progress and modifies residents' individual care plans, in collaboration with the Life Enrichment Director, care coordinators and neighborhood team members.
Attends care conferences for residents in Wildflower Way, and other resident planning meeting and conferences, as needed.
4.
Customer Service
Responsible for creating a culture that incorporates the Eden Alternative principles.
Embraces resident individuality and meets each resident at their ability level.
Works closely with residents, their families and friends, and neighborhood teams to assure optimal opportunities for residents to lead joyful, purposeful lives.
Works with all disciplines to promote and provide opportunities for residents to engage in normal pursuits as well as promoting successful and well-balanced leisure lifestyles.
Integrates “Going Further, Together,” principles in all interactions, including AIDET, 5/10, and managing up.

This job may include other duties as assigned to meet the needs of the residents and the organization.
Job Requirements:
Minimum Education
Required: HS graduate with two years vocational training/college degree or related work experience.
Preferred: 4-year college degree in therapeutic recreation or similar field such as social work, parks and recreation, wellness education.

Minimum Work Experience
Required: Previous position that utilizes creativity, organizational and people skills.
Critical to have compassion for and desire to work with the elderly.
Ability to communicate effectively with residents, families, staff, and the general public.
Preferred: 2 years of experience in a social or recreational program role with at least 1 year in a health care setting.
Prior experience in a long-term care facility.
Required Licenses: BLS required at the time of hire.
Also prefer certification of: Certified Activities Professional or Certified Therapeutic Recreation Specialist.
